001. Fatty change, liver
Etiology : alcoholism､obesity､malnutrition､chronic wasting disease﹐D.M.﹐Chloroform﹐CCl4 .
常發生於 Liver﹐heart.
Gross : 肝腫大､切面呈黃色､油漬狀。
肝細胞的細胞質內含有 fat vacuoles﹐可溶合成大的 clear space。
細胞核被 fat 擠至 cell 的周邊部份。
肝細胞因 fat vacuoles 而漲大。
002. Vacuolar nephrosis, kidney
Etiology : chloroform or CCl4 中毒﹐infection, high fever, & hypocalcemia。
mechanism : 由於 cell 無法保持水分及離子之平衡﹐細胞外之水分進入細胞內造成 cell swelling。
常發生於腎臟的 proximal convoluted tubules 之上皮細胞﹐亦可見於 heart及 liver 之 parenchymal cells。
micro下可見近曲小管之上皮細胞較為透亮(因為水分積聚)。
在胞漿中亦可見小而透明之 vacuoles﹐有時 nucleus 會被 vacuoles 擠至一旁。
003. Polycythemia, marrow
Cellularity 約75%﹐各系列的造血細胞皆有增加﹐尤以 Erythroid 系列中之normoblast 增加最為明顯。
可見成叢的 megakaryocytes﹐並可見 Giant megakaryocyte。
與 CML 及其它的 Myeloproliferative Disease做鑑別診斷。
004. Hyaline degeneration (in chronic membranous GN), kidney.
因過多之 protein 經由 glomerulus 濾至腎小管﹐由於近曲小管上皮細胞的胞飲作用而在其胞漿中形成 pink hyaline droplets。
Micro下可見腎小管上皮細胞中有 pink hyaline droplets。
006. Brain edema, Brain.
Edema : 過量之體液堆積於胞內或細胞間。腦水腫通常是由於腦部之外傷﹐感染﹐腫瘤﹐梗塞﹐腦部靜脈回流受阻等引起。
Micro 下 Brain tissue 較疏鬆﹐液體在細胞內和 interstitial space 堆積。
在小血管周圍的 peri-vascular space 或細胞內細胞核旁空間變寬﹐造成 clear halo。
011. Old infarct, Brain
一般器官之 ischemic necrosis 最常見是 coagulative necrosis﹐而在 brain 則是 liquefaction necrosis。
在 micro 下﹐受影響之區域顏色較淡而且疏鬆﹐由於有 edema ﹐而使interstitial space 變大。
受傷地區亦會有很多 microglia 聚集行 phagocytosis ﹐可見macrophages 吞噬 cellular debris 及 hemosiderin pigment。此種細胞又稱 gitter cell。
013. Amyloidosis, kidney.
在 glomeruli 中有 homogeneous, eosinophilic, hyalinized substance ( amyloid ) 堆積。 glomeruli 之架構都己經破壞。
除了 glomeruli 外﹐arterial & arteriolar walls 也受影響。pinkish hyalinized thickening of arterial walls﹐管腔變窄。
有些區域連 peritubular tissue 也有 homogeneous, eosinophilic, hyaline deposit。
可見血管有 congestion 現象。
round cells infiltration。
若用 Congo red 染色 amyloid 會染成紅色且在偏光顯微鏡下觀察有Birefringence 的現象﹐可與其 protein 之堆積區分。
019. Chocolate cyst, ovary
Chocolate cyst﹐多因為 endometriosis of ovary 造成。
endometriosis of ovary 可在 ovarian parenchyma 中找到 endometrial gland and stroma﹐其中 endometrial stroma 可造成出血､發炎､及 adhesion。
發炎愈嚴重則愈不易找到 endometrial glands 或 stroma﹐最後形成一個 cyst 含有 blood 及其分解物。由於呈褐色故稱為 chocolate cyst。
Micro 下﹐可見 cyst wall 上有數層 hemosiderin-laden macrophage 及 fibrous tissue。
021. Metastatic calcification, kidney (Renal calcinosis) (focal pyelonephritis with stag-horn stone)
Etiology : hypercalcemia。
在 epithelial cells of renal tubules 有basophilic amorphous deposition of calcium salts。
focal round cell infiltration。
030. Nodular goiter, thyroid.
特徵為 nodularity. Nodules 大小不一﹐其間有 fibrous septa. Nodule可完全或部分被 encapsulated.
有些 Nodules 會有 degeneration 情況﹐產生 cholesterol crystals﹐calcification等。
也有 hemorrhage with hemosiderin deposition﹐fibrous scarring﹐或者 cystic degeneration.
follicle 之 epithelium 大小不同﹐有的正常﹐有的變平﹐有的變高。
032. Congestion & edema, lung
可因 heart failure, renal disease, alveolar wall 破壞或 infection造成。
congestion : alveolar septa 變寬﹐alveolar capillaries 擴大﹐有輕微之 intra-alveolar hemorrhage﹐有時可見hemosiderin-laden macrophage.
edema : 在 interstitium 及 alveolar wall 有 protein-rich fluid 聚積﹐有時 edematous septa 會 fibrotic 或有 hemosiderin pigmentation 稱為 brown induration.
長期 congestion 下﹐會使 pulmonary arteries & arterioles 之管壁變厚。
部分區域有 round cell infiltration.
036. Centrilobular congestion and necrosis, liver.
由於 right side cardiac decompensation 造成 congestion of central vein 及 centrilobular sinusoids﹐使 centrilobular congestion 部分呈紫紅色﹐而外圍則為正常之 liver lobules﹐ 此現象gross 稱為Nutmeg liver.
可因 centrilobular congestion or hypoperfusion hypoxia 引起rupture of capillary sinusoid �_ hemorrhage �_ 造成 centrilobular liver cells 之 compressive atrophy 及 necrosis.
central vein 及 centrilobular sinusoids 有 congestion﹐及centrilobular region 有 hemorrhage.
centrilobular lover cell 破壞消失。
micro下﹐可見 hemorrhagic necrosis of centrilobular area.
037. Cerebral hemorrhage
常因為 hypertensive vascular disease, trauma, rupture of aneurysms, angioma, blood dyscrasias﹐及 tumor 引起。
腦組織己經部分破壞了﹐而且為血塊所取代﹐這跟 hemorrhagic infarct不同。
在 hemorrhage 周圍之腦組織有 edema 現象。
周圍的腦實質中也有明顯的 perivascular hemorrhage.
血管可見 congestion.
040. Disseminated intravascular coagulation, kidney
主要為 fibrin 及 platelets 聚成 microthrombus 廣泛地分布於microcirculation 中。
fibrin, RBC, platelets 形成 microhtrombi﹐存於 capillary loops of glomeruli.
有 focal tubular cell necrosis.
041. Acute suppurative appendicitis.
在 mucosa, submucosa, 及 muscularis propria 均有 neutrophil infiltration.
subserosal vessels 有 congestion﹐其旁亦有 neutrophil infiltration.
044. Chronic sinusitis
mucosal tissue 有明顯的 edema 及 hyperemia.
以 eosinophils & plasma cells 為主之 chronic inflammatory cell infiltration in mucosal tissue.
stroma edema, fibrous tissue �^ ﹐故 hypertrophy.
有 nasal polyps formation.
由於3 & 4﹐故病人易鼻塞。
046. Brain abscess
可能由於 trauma 使 organism 直接進入﹐或由其他鄰近地區之感染擴展而來﹐或由遠處感染經由血路轉移引起。
低倍鏡下可見一片藍色區域是由 leukocytes 及 cell debris 所組成。
整個 abscess 之 lesion 分二層 :
內層 : cell debris﹐inflammatory cells (主要為 PMN).
外層 : fibrosis and gliosis 及血管增生。
047. Purulent meningitis
早期有血管之變化 : hyperemia﹐局部血管旁出血﹐cellular exudate出現﹐更嚴重會有 thick purulent exudate.
在 leptomeninges 有 dense inflammatory exudate﹐主要由 PMN leukocytes 及 fibrin 所組成。
meninges 的血管有 congestion。
neuron 產生 ischemic change ( red neuron ) .
051. Granulation tissue, Skin
皮下組織和真皮被 granulation tissue 所取代。
small newly formed blood vessels和young fibroblasts 增生 granulation tissue.
Edematous Change.
Infiltrated with acute inflammatory cells, macrophages, lymphocytes, and plasma cells.
053. Chronic gastric ulcer.
Interuption of mucosa.
慢性發炎細胞的浸潤。
Ulcer 主要分為四個 Zones :
在 ulcer 的底層和邊緣有一淺層的 necrotic fibrinoid debris.
再下一層是非特異性發炎細胞的浸潤。
更深一層有 active granulation tissue 和單核白血球的浸潤。
最下一層則是 fibrous 或 collagenous scar.
圍繞在 gastric ulcer 周圍的胃黏膜通常很多發炎細胞的浸潤 : 有plasma cells, lymphocytes 和一些 neutrophils.
054. Myelofibrosis, bone marrow
本片可見大量纖維組成, 纖維細胞核為尖形, 有粉紅色絲狀細胞漿, 細胞界限不明顯.
Megakaryocyte 增生, 其形態具大量粉紅色的細胞漿和巨大,不規則的核.
纖維組織間亦雜有少量紅血球和白血球等造血細胞.
067. Adenocarcinoma, Endometrium.
可有不同之分化程度
Grade 1 ( Well differentiated ) _ 顯著且易於辨認的腺體構造。
Grade 2 ( Moderately differentiated ) _ 腺狀構造和 solid sheets of malignant cells 相互混合。
Grade 3 ( Poorly differentiated ) _ 主要為 Solid sheets of malignant cells 和一些幾乎無法分辨的腺狀構造﹐nuclear atypia, mitotic figure。極為明顯。
此片屬於grade 2.
多成腺體狀排列﹐anaplasia 明顯。
Invasion of myometrium, focal.
tumor cell 在blood vessel 中 ( Tumor emboli ) .
071. Candidiasis, thyroid.
在組織中 Candida 沒有分枝﹐像有蓋貨車 ( boxcar ) 般的一串串filament﹐叫做 pseudohyphae.
Yeast form 約2~4μ﹐由 pseudohyphae 芽狀突起﹐叫做 blastospores, Gram’s, PAS 和 silver stain 可更清楚地染出。
micro 可看到 thyroid tissue 很嚴重地被許多 fungi 侵犯所破壞。
此片沒什麼發炎反應﹐與病人的 immunocompromized state 有關。
072. Pulmonary Tuberculosis with cavitation & aspergillosis
Aspergillus 的特點 :
- Branching filaments 5
到10μ 厚﹐有 septation.
分支角度小於90°。
其 fruiting body ( Conidiophores ) 很像 holy water sprinkler.
傾向於侵犯血管﹐所以除了 suppurative tissue reactions 外﹐也有局部的 hemorrhage和infarction.
此片可看到 pulmonary congestion, edema, hemorrhage 外﹐也有許多caseous tubercles 和 granulomas 及 giant cells.
有個 Cavity﹐其內有 necrotic tissue, RBC, inflammatory cells 外﹐可看到 aspergillus 的 ” fungus balls “﹐可看到菌絲﹐外面由granulation tissue 和 dense collagen fiber 所包圍。
Aspergillosis的反應可分三種 :
(1) Allergic (2)Colonizing (3)Invasive
其中 Colonizing fungus 長於 pulmonary cavities 裏﹐很少或不侵犯到組織﹐這些cavities 常伴存先發生的TB, bronchiectasis, old infarct 或abscess 中。在cavities 中找到 fungus hyphae 的增生團 “ fungus ball “ 其旁的reaction 可以是沒有﹐也可以有 Chronic inflammation 和 fibrosis.
073. Cryptococcosis, brain.
Causative agent : Cryptococcus neoformans
圓形﹐橢圓形的 yeast 約 5~10μ大﹐外圍一圈很明顯的 gelatinous capsule﹐因其不易著色使它在組織切片中或 exudate 中呈 Clear halo.
一般它會造成 Chronic granulomatous reaction﹐包含有 macrophage, lymphocytes, foreign body type giant cells, neutrophils﹐有時也有 suppuration, 也可以沒有炎症反應。
Cryptococcus infection 最常見的表徵是 meningitis﹐有一半的case會侵犯到腦﹐大都是在 Virchow-Robin (perivascular) space﹐有時進入到 gray matter 造成 small cysts﹐裏面充滿 Cryptococcus和它們的 mucinous secretion 造成像 “ Soap-bubble “的 lesions。
Subarachnoid space 中可看到許多大小差異的 Cryptococcus microorganism。
brain 中也受到影響﹐在 peri-vascular space 中有許多 cyst﹐其內充滿 Cryptococcus 和它們的 mucinous secretion.
沒多少發炎反應。
077. Bacterial vegetation, mitral valve
mitral Valve 上 coated with purulent exudate 與 thrombus, valve結構破壞､有 necrosis﹐在 exudate 中夾著大量 bacterial colonies。
Valve 之外的 endocardium 無炎症波及﹐myocardium 的 myocytes 有點 hypertrophy 與明顯的 sarcolysis。
081. Cytomegalovirus inclusion disease, lung.
Causative agent：CMV, a member of the herpes virus group.
典型的 CID，特徵是相當大的細胞裡出現 Amphophilic intranuclear inclusions，這個 inclusions 可有細胞核的一半大，旁邊圍繞一圈Clear halo，使得它和細胞核模很清楚的分開。
也可以看到 Amphophilic smaller cytoplasmic inclusions.
Inclusions 和 focal necrosis 的情況可發生於許多器官，但以salivary gland, kidneys, liver, lung, guts, pancreas, thyroid, adrenals, brain 較常見（依序排列）。
在 Lung 的 變化有 interstitial pneumonitis, intraalveolar edema, proteinaceous exudate, focal hyaline membrane；intracellular inclusions 可以在 alveolar的lining cells, septal capillary 的endothelial cell 和 alveolar macrophage 發現。
interstitial pneumonitis。
alveolar septa 變寬，有 round cells infiltration。
局部 Intra-alveolar hemorrhage和exudate。
高倍鏡下看到特別大的 alveolar epithelial cells，內含一個很大的 pleomorphic nuclei，核內藏一個圓形的 intranuclear Amphophilic body，外圍一圈 Clear halo， 是 Cytomegalovirus inclusion body。
這種細胞在本片中不是很多，但也不難找到。
091. Hyperacute rejection in renal transplantation.
Renal transplantation的hyperacute rejection，在 microscope 下主要看到 large-sized and medium-sized arteries 內有 thrombus formation 而引起 renal infraction.
在這 slide 中可看到 arcuate 及 interlobular arteries內塞著 thrombi.
血管壁、renal tubules及 glomeruli 等尚未有明顯的變化。
在renal cortex處可看到一個 Simple cyst，此為一 incidental finding，並無特別意義。
092. Choroid plexus papilloma.
Papillary structures 一單層之 columnar 或 cuboidal cells lie upon a basement membrane that covers a delicate vascular connective tissue.
雖有 tumor necrosis，但細胞分化好，沒有 mitoses.
Stroma 中偶有 calcification.
093. Parkinson
’s disease
在大腦後面的 Substantia nigra（確實位置應在 Zona compacta）中可見很多 neuron 被破壞。
Neuromelanin pigment 之顆粒釋放，散落在 neuropil中。
Gliosis.
Lewy body 出現在一些剩下的neurons 中。Lewy body是intracytoplasmic，eosinophilic round- to-elongated inclusions，中間染色較紅，外面有一圈 halo 圍著。
095. Malignant lymphoma, nodular, predominant small cleaved cell(Lymph node)
本片正常的淋巴腺結構已經破壞，淋巴腺中之 cortex、medulla 及外圍之 soft tissue 低倍下可見圓形或橢圓形之 nodules. Nodules內為lymphoma 細胞，其中大部分為 small cleaved cell.
Small cleaved cell： 細胞無核仁，有點狀的染色體，核膜不規則，細胞漿少，細胞界線不明顯，細胞分裂像很少。
少部分 lymphoma cell 有較大的細胞核。
100. Primary diffuse hyperplasia (Graves
’ disease),Thyroid gland.
Follicles 皮細胞變成較高的 cuboidal 或columnar 狀，細胞數目增加，造成 papillary buds 突起到 follicles 中，這些 simple papillae是 nonbranching，比旁邊上皮細胞高些，有些像蕈樣的突出，細胞大小形狀一盡相同，但無atypical 的現象。
Colloid 顏色變得淡些，邊緣有 vacuolation 的現象。
follicles 間的 stroma 有 patchy lymphoid infiltration 現象，並可伴有lymphoid follicles 的生成。
Interstitial 間有些部位血管多似 congestion.
以上諸現象，可因外科準備開刀，用藥的關係而有所改變：如 iodine 可促使 colloid storage，devascularization，gland involution；而thiouracil會造成厲害的 hyperplasia.
101. Testis, atrophy
Seminiferous tubules的basement membrane 很厚(有 hyaline change thickening).
有些 Lumen 呈 hyalinization 而無細胞，其餘的也僅存一些 Sertoli cells，幾乎沒有 Spermatogonia.
Interstitial cells (Leydig cell)增生。
103. Squamous metaplasia, cervix
這片位於 squamocolumnar junction
Cervix gland 上 的 covering epithelium 本應是 columnar epithelium，不過現在是 Squamous epithelium.
Metaplasia 與 dysplasia之區分：
Metaplasia細胞排列較規則，且層數不會太多，約5-6層，細胞胞漿多，核染色淡，無明顯 atypism.
102. Nodular hyperplasia, prostate.
在較低倍或肉眼下可以看到結節狀，此為 stroma 或腺體增生的結果。
腺體增生，數目增加密集，細胞較大或層次增加。
腺泡由兩層細胞所構成，內層為 Columnar 而外層為cuboidal 或flattened；並可向 lumen 內突出形成 papillary buds 和 infoldings.
通常腺體內還包含：
(1)Inspissated secretion
(2)Desquamated epithelial cells
(3)Numerous corpora amylacea
104. Dysplasia, cervix
Squamous cell epithelium 細胞層數很多，下層的細胞排列尤其不規則，細胞形狀不一，不成熟，有一些mitotic figure.
較下層的細胞有明顯的 nucleocytoplasmic ratio 的改變，loss of polarity，pleomorphism 等atypical的現象。
依程度不同，可有very mild、mild、moderate 及 severe dysplasia之分別。
106. Pituitary adenoma.
Patternless sheets of cells with round nuclei and indistinct cytoplasmic borders.
細胞間有 delicate septa of connective tissue and vessels.
Elongated cells 垂直於 fibrovascular septa 排列 (sinusoidal type).
Papillary appearance_起於 artifactual separation of the tissue.
大部分細胞為 chromophobic-clear cytoplasm.
107. Parotid gland, Pleomorphic adenoma
低倍鏡下，已見正常涎腺腺泡圍繞著一瘤塊。
高倍下，瘤塊內有明顯的 cartilaginous tissue，並見 myxoid 及hyalinzed 間質組織。myxoid tissue_abundant basophilic ground substance.
腺管上皮組成的腺管狀構造，腺管大小不一，內層為低柱狀上皮細胞，外層為扁平上皮細胞(myoepithelial cell)。Epithelial cells_cuboidal or columnar，或含有 mucin-containing vacuole；Myoepithelial cells_smaller, darker, tend to be polygonal，with eccentric nucleus, and hyaline eosinophilic cytoplasm (myoepithelial cells are usually not easy to identified in H&E stain).
瘤塊外圍包有緻密的組織莢膜。
108. Hepatoma, liver
本 case 的癌細胞較109片之分化好，細胞及胞核之大小有輕微差異，但與正常肝細胞相像。
瘤細胞多呈大而寬的束排列 (trabeculae) ，每束間均有的血竇(sinusoid) 且有endothelial lining.
正常的 Portal area 及 central vein 之關係消失。部分 hepatocytes有明顯的 bile stasis.
有 postnecrotic cirrhosis.
Tumor emboli in portal vein of portal triads.
110. Adenocarcinoma of cardia of stomach.
本片取自胃的賁門區域，在低倍鏡下可看到正常的食道鱗狀上皮，其黏膜下靜脈有充血現象。
賁門的黏膜有 tumor，由腺上皮形成，細胞核大而濃染，核仁明顯，腺狀結構混亂且大小不一，並向下侵犯破壞胃壁肌肉及漿膜，部份胃黏膜也見潰瘍。
Tumor cells 都有明顯的 tubular or glandular structure 的分化，為一 intestinal type 的 adenocarcinoma.
111. Poorly differentiated adenocarcinoma, stomach (Signet-ring cell).
Tumor cells 不形成 tubular or glandular structure，而係 diffusely infiltrating 在entire wall，為典型的 diffuse infiltrative type (Linitis plastica).
很多 tumor cells 呈 “signet-ring” appearance.
Signet-ring cell 的胞質內含大量黏液並把細胞核向邊緣擠壓，因而構成特殊的細胞形象。 The tumor cells infiltrate the upper part of mucosa (superficial spreading), therefore the early diagnosis is difficult.
114. Compound Nevus
在 upper dermis 有 clusters and nests of nevus cell 為 intradermal nevus.
在 epidermis 與 dermis 與 junction 也有少數 nevus cell nest，稱junctional nevus.
很多 nevus cells 被 melanin pigment 遮蓋以致看不清楚它的胞核，但基本上這些 nevus cells 的胞核都還溫和。
119. Warthin
’s tumor, Salivary gland
此腫瘤組織型態特殊，合上皮細胞與淋巴基質而成。上皮部分形成cystic spaces，故又稱cytadenoma lymphomatosum.
上皮細胞有兩層：
(1)上層細胞為柱狀且嗜酸性(像oxyphilic oncocytes)
(2)下層為polygonal或pyramidal shape cells.
二層細胞重疊並排列成乳突狀構造。
不規則的囊腔內含有嗜伊紅性分泌液。
間質內有大量淋巴球所組成的基質，並會集結成為胚中心(germinal center).
瘤塊外包有纖維莢膜。
122. Epidermoid carcinoma in situ, cervix
細胞層數增加。
整層的 epithelium 受影響，loss of polarity，細胞形狀不一，並出現pleomorphism 和 mitoses.
Basement membrane 仍完整，無 invasion.
可有 glandular involvement，但仍在 basement membrane 界線之內，故仍屬 CIS。
123. Adenocarcinoma, colon.
大腸的 Adenocarcinoma 與胃，小腸等的 adenoCA 組織型態上差異不大，均以腺管結構為主。
本片可看到正常的黏膜高柱狀上皮細胞過渡到癌細胞的情況。
癌細胞雖保存柱狀外形，但胞核濃染， chromatin 粗而明顯，分裂較多。
腺管結構大小不一，而且不規則，管腔內多充滿黏液，甚至成為 mucin pool.
癌細胞已穿透整層的大腸壁，而到達了 paracolic soft tissue.
130. Medullo(myo)blastoma
medullomyoblastoma 與 medulloblastoma 相似，較易出現 subarachnoid seeding of tumor cell，此例是小腦的medullomyoblastoma 經過 subarachnoid seeding 後到達 Dura.
顯微鏡下與 medulloblastoma 類似。
此例在 Primary tumor 裡找到 striated muscle cell，故稱medullomyoblastoma，但此片 Dura 並沒找到。
131. Schwannoma
此 tumor 發生在 peripheral nerve
顯微鏡觀較 No.121 high cellularity, nuclear palisading 明顯
Palisading nuclei 成 whorl formation
Verocay body --
elongated tumor cells 成長索或 圓形的聚集成團，其中嗜伊紅性的細胞質位於中間而 Palisading nuclei 排列在週圍。
138. Germinoma
tumor 通常在 gross 下可見到 cystic foci, hemorrhage 和 calcifications。
一大片單一且規則的 tumor cells，在圓形的細胞核外圍有 Clear cytoplasm 呈一 clear halo ，如 fried eggs appearance。
偶而可見 giant cells 和 endothelial proliferation。
Calcifications (both in tumor and its vicinity)
139. Glioblastoma
cellularity 顯著增加。
pleomorphism 甚為明顯。
出現 mitotic figure。
出現 endothelial proliferation。
出現 tumor necrosis (frank or pseudopalisading type )。
Hemorrhage。
bizzare giant cells 很多。
142. Fibrocystic disease
Fibrosis of the stroma.
Duct 增生及 dilatation，形成 cysts. Cyst 的上層多為單一或雙層。
bsaement membrane 仍完好。
lobules are atrophic and distorted by stromal fibrous proliferation.
143. Condyloma acuminatum
因為位置不同，雖有相同的 histology 變化，卻有不同的名稱：
- verruca vulgaris -- hands (elevated)
- verruca plantaris -- sole of foot (callus)
- Verruca plana -- face and hands (flat)
- Condyloma acuminatum --around the anus, vulva, penis
Micro 下：
- Epidermal hyperplasia
- Hyperkeratosis
- Parakeratosis
- Acanthosis
- Papillomatosis (
在 Verruca plana 無)
Vacuolization of the cytoplasm of keratinocytes；此為早期之變化，晚期則不存在。
這種細胞有 pyknotic nuclei，perinuclear halo，和一薄層的細胞質-- koilocyte
在 dermis 可有 inflammatory reaction

149. Infiltrating duct carcinoma, breast.
Individual tumor cell：
大小、外形不一，具分裂像。
細胞 -> 大小外形不一。
Tumor cell 有 Central necrosis. papillary or cribriform pattern.
Tumor cells 穿出 basement membrane 到 connective tissue -infiltrative growth 而成一單層的細胞排列(正常的 duct 有兩層細胞)。
increased dense fibrous tissue stroma.
The stroma is infiltrated with lymphocytes and plasma cells.
151. Fresh myocardial infarct, heart.
在 infarct 處，週圍有的肌纖維呈波浪狀(waviness)而且有斷裂之現象。
neutrophil infiltration 非常明顯。
有 coagulation necrosis，細胞核 pyknosis, or karyolysis, with eosinophilic cytoplasm.
有的核消失或肌纖維之橫紋消失。
edema，及 congestion。
papillary muscle 有 old infarct。
152. Coronary artery atherosclerosis & myocardial hypertrophy
Coronary a.內可見到一個半月形之 atheroma，其內有 calcification 之現象。
Advanced atherosclerotic plaque；外覆以 endothelium，其下為 fibrous cap (包括增生的平滑肌細胞、collagen、lipid 及 foamy cell)，最中央為 necrotic center (包括有 cell debris, cholesterol crystal, cholesterol esters, calcium)
血管之 media 有點萎縮。
Myocardial hypertrophy；細胞變大，核也變大，而且有的細胞核形狀成多角形。
161. Atherosclerosis of aorta
intima 增厚：lipid 堆集；fibroblast, smooth muscle 增生，internal elastic lamina 增生及斷裂。
media 退化；有 hyaline 及 myxoid degeneration of the smooth muscle。
可見 cholesterol crystal 及 calcification。
*4.adventitia 已 fibrosis 並有 chronic inflammatory cell infiltration。
163. Vascular malformation, brain.
在 brain 中之 vascular malformation 可分為三種：arteriovenous malformations (AVM)， capillary telangiectasia，及cavernous angiomas。三者各有不同之特徵。
本片主要為 AVM，可見有 tangles of abnormal vessels 及不同 size 之vessel channels of arteries & veins，有些血管據動、靜脈的特質。
血管由 gliotic brain tissue 分開。
165. Capillary hemangioma, skin
多見於 skin subcutaneous tissue, oral cavity & lips 之 mucosa，若發生於內臟則常見於 liver, spleen, kidneys。
可見一堆密集之 vessel，壁薄，大小如 capillary。其 endothelium 完好。
血管間由 connective tissue stroma 分隔。lumen 內有紅血球(與 lymphatic duct 有別)。
166. Centrilobular emphysema, lung
Emphysema 之定義為 acinus 之永久性擴大及有 alveolar walls 之破壞，主要可分為 Centriacinar (centrilobular)，panacinar，paraseptal(distal acinar)，及 irregular emphysema 四種。
Centrilobular emphysema 就是影響到 central or proximal parts of acini，所以可見到 respiratory bronchioles 附近之 acini 擴大較厲害，較遠處之 acini 則不受影響。
Emphysema lesion 附近的 alveolar 被壓迫成 slit-like pattern。
在 emphysematous walls 上可見 anthracotic pigment。
168. Bronchioloalveolar carcinoma
可見到 columnar or cuboidal epithelium 沿著 alveolar wall 生長，為此 tumor 之特性，Cancer cell 分化好，核多位於底部。
有時 tumor cells 也會伸入 alveolar space 而形成 papilla，有時也可看見 multinuclear giant cell。
172. Small cell carcinoma (Oat cell type)
組成此 tumor 之 cells 小，約為 lymphocyte 之二倍，形狀由圓形至短梭形，細胞漿少或不明顯，細胞核濃染。
細胞各自分散，無特殊結構，有時會以一叢或一團的生長，但無腺狀結構。
此例伴有明顯的 necrosis。
174. Lobar pneumonia
lobar pneumonia 通常都影響到一整個 lobe 或整個 segment。少發生於小孩及老人，常由於 bacteria infection，可分為四期，congestion，red hepatization，gray hepatization，resolution。
micro 下，可見幾乎所有 alveolar space 都充滿 neutrophils 為主之 acute inflammatory exudate，也可見少量之 fibrinous exudate。
175. Nasopharyngeal carcinoma(NPC), undifferentiated
此為一分化差的 epidermoid carcinoma。
Tumor 呈 solid sheets 或 nests 排列，細胞漿為 eosinophilic，細胞界限不明，細胞核為短梭形且呈 vascular 狀。
177. Interstitial pneumonia
本片為 measles 引起之併發症，與其它 viral pneumonia 一樣，也為interstitial pneumonia。
alveolar wall 由於 inflammation 及 fibrosis 而變厚，可見有macrophages 及 mononuclear cells infiltration。
peribronchiolitis 很明顯，可見明顯的炎症細胞浸潤，可以看到散在之giant cells。
在 bronchi 中有 acute inflammatory exudate，可見有secondary bacterial infection。
請自行比較 slide 174,178。
有 type Ⅱ pneumocyte hyperplasia 及到處有 congestion 現象。
180. Osteosarcoma
Osteosarcoma 的主要特徵為由 stromal tumor cells 產生之 tumor osteoid 或 woven bone；此 osteoid 散布於 stromal tumor cells 間，為 eosinophilic，glassy，細長而彼此交織在一起，像 lace 或 stream 一樣，有時會 calcification。
Stromal tumor cells 可類似 osteoblast，chondrocyte 或 fibroblast 形狀很不規則，核濃染，有時可見 tumor giant cells 及 atypical mitosis。
Osteostroma 常穿透 periosteium 而侵犯軟組隻如橫紋肌等。
181. Mesothelioma with hemothorax
此為 malignant mesothelioma，其中一塊含有 epithelium 成份之tumor tissue 為主要部份。其餘的 fibrous tissue 有出血及發炎現象。
Tumor tissue 中可見到 whorls of reticulin 及 collagen fibers，而在其中又有似 fibroblast 之 spindle cells。
這些 fibrous tissue 及 mesenchymal stroma 及 epithelial cells，都由 mesothelial cell 分化而來。上皮呈多邊形，細胞漿紅色，雖為惡性，但細胞核很規則，看起來很溫和，細胞呈 tubular、nests 或 sheets 排列。
189. Ewing's sarcoma
本例已經過治療，可見有 repair 之 bone fibrovascular tissue，但在preserve 較好之區域，仍可見較典型的 Ewing's stroma。其組成的細胞為primitive small round cell，Tumor cells 無特殊排列，細胞漿界限不清楚，核在 preserve 較好的區域可見是 vesicle 的。細胞很 uniform。
lobular 排列是 Ewing's stroma 之特徵。在 lobules 之間為 fibrous septa。
Ewing's stroma 屬 small round cell tumors 之一種，其它較常見的包括有：lymphoma, rhabdomyosarcoma, neuroblastoma, poorly differentiatedcarcinoma 等，常常除了 LM 之外還要 EM，才可診斷。或是用 PAS stain 可染出於細胞中之 glycogen 作鑑別診斷。
191. Amebic colitis, colon
本片 colon 之 epithelium 有一部份已經脫落，ulcer 已經侵犯至肌肉層。
有很厲害之 necrosis 及 inflammatory reaction。
在 ulcer 中或其 base 可找到 ameba。submucosa 及 serosa 下可見到congestion，serosa 有 fibrinous exudate。
40% 之 ameba 會侵犯血管，流至 liver 造成 abscess。
194. Ameloblastoma
在 micro 下，tumor 已經侵犯至 bone marrow 處，bone trabecular 明顯的被吸收。
Tumor 之組織形態與 tooth bud 很相似tumor nests 之外層為排列整齊之 柱狀上皮細胞。內層為疏鬆之星狀細胞。與 tooth bud 之 ameloblastic epithelium 和 stellate reticulum 非常相像。Squamoid differentiation is occasionally found at the part of tumor.
此 tumor 以 local invasion 為主，很少有 metastasis，但常因手術切除不完全而易再發。
195. Adenomatous polyp, colon
Adenomatous polyps 為一種 neoplastic adenoma，又稱為 tubular adenoma，若合併有 villous structure 則稱為 tubulovillous adenoma。
本片在肉眼下即可見黏膜層長出有 stalk 之凸出物即為 polyp。
micro 下，polyp 之底部還是正常之大腸黏膜，但在頂部，上皮細胞及腺體之排列很密集，數目也很明顯地增多。細胞核大而濃染，胞質內 mucus 減少，有 mitosis，但數目不多。
polyp 中央為 fibrovascular core，有點 congestion。
206. Malignant lymphoma, diffuse, large cell.
本片為胃旁之adipose tissue被惡性淋巴浸潤。
細胞以大型的lymphocytes為主 ( large non-cleaved cell ) ，少部份為large cleaved cells及small cleaved cells.
可找到mitosis.
207. Malignant lymphoma, lymphoblastic, Lymph node.
因specimen中央固定不好，所以要觀察外圍的細胞。
淋巴腺結構已破壞，有小的惡性淋巴球浸潤，細胞漿少細胞核為圓形，部份核膜不規則，有小型之核仁及細少之染色體。
分列相 (mitosis) 多。
lymphoblastic lymphoma可分為convoluted及unconvoluted兩種type.
212. Portal cirrhosis, liver
You may see:
- (1)micronodules dissected by delicate fibrous septa, pericellular fiber.
- (2)extensive fatty change of parenchyma within the nodules,
- (3)lymphocyte infiltration in portal area.
- (4)Bile stasis in hepatocytes like carol.
- (5)Mallory body
215. Submassive hepatic necrosis
在低倍鏡下可見到肝小葉的邊緣，已經有明顯的壞死現象及炎症細胞浸潤。
已經損壞之 reticulin framework 及 portal tract 仍然可見。Bridging hepatic necrosis (central to central; central to portal)
Bile ductules 增生及 bile stasis 都可見到。
Bridging hepatic necrosis (central to central, portal to portal) 。
217. SJogren's syndrome, Salivary gland
Sjogren's syndrome 是 autoimmune disease 的一種。其主要影響器官包括腮腺、淚腺及關節囊。
本片下，可看到 typical 的顯微變化如 salivary parenchyma atrophy；非常嚴重的 interstitial lymphocytic infiltration 並形成大小不一的lymphoid follicles。
腺管的肌上皮細胞增生成十分活躍的 "myoepithelial cell islands."
219. Biliary cirrhosis & Schistosomiasis, liver
Biliary cirrhosis 是由於 bile ductule 或 hepatic duct 受損而造成bile 外流，引起肝硬化。可分為 primary 及 secondary biliary cirrhosis.
micro 下之特徵主要在 regular scarring 及嚴重之 bile stasis。
低倍鏡下有明顯之 periportal fibrosis，fibrous septa 將 portal triads 互相連接，而且在 septa 中有 mononuclear cells infiltration 及 bile ductule proliferation。由於免疫反應破壞 bile ductules 之上皮，所以可以在 fibrous septa 中看見 tangled knots。
被分割一片片之肝小葉中，可以見到肝細胞及 canaliculi 中有 bile stasis。
有時可見到 lymphoid follicle 及 foamy histocytes。
在 fibrous septa 中同時可看見鈣化之 schistosoma 的蟲卵。
223. Cholangioma, liver
又可稱為 cholangiocarcinoma，多為分化良好之 desmoplastic adenocarcinoma。
tumor 主要是腺狀或管狀結構，上有 cuboidal 或 columnar epithelium lining。因為屬於纖維間質多之 tumor，所以可以看見 dense collagenous stroma 分隔 cancer cells。
可在 tumor 中見到 microcyst，有的甚至有乳突狀結構突出於 cyst 中，其上皮亦為立方或柱狀上皮。同時在 cell 或 lumen 中可見到 mucin。
肝實質被瘤破壞有出血及 bile stasis; centrilobular cholestasis。
Bile ducts have CIS or dysplasia; Centrilobular cholestasis。
226. Dissecting aneurysm, aorta
cystic medial necrosis 為最常見的原因。
主要病變在 tunica media，低倍鏡下可以看見 media 內2/3與外1/3交界處內有很多 RBC。
高倍鏡下可見 media 之 cellularity 減少，elastic fiber 與平滑肌纖維減少，而增加了嗜鹼性之 ground substance of connective tissue (本片不明顯)。elastic fiber 斷裂、凌亂也是特徵之一。
229. Avascular necrosis, Femur head
Bone 之 necrosis，bone marrow 最早發生變化然後是 bony trabeculae。
本片可見 marrow-element 已經失去正常之結構只剩下粉紅色的granular material : granular necrosis。
bone 之 necrosis 則為骨細胞消失，lacunae 中之 osteocyte 已消失(empty lacuna)，但骨頭的 matrix 變化不大。
234. Fat necrosis (acute hemorrhagic pancreatitis)
mechanism：由於活化之胰酵素流入胰臟的 parenchyma，而引起 pancreas 之 interstitial inflammation & tissue digestion。
fat necrosis--fat 呈透明空泡狀 ，可產生 calcification，在某些地方已成 homogenous pale pink material。
有 congestion & hemorrhage -- 由於血管發生 necrosis 導致 hemorrhage。
有 inflammatory reaction-leukocyte infiltration 於壞死區域週圍。
有 hematoidin (血棕晶質)沉積於 fat necrosis 之邊緣。
235. Benign & malignant nephrosclerosis, simple cyst
及 microadenoma,Kidney
malignant nephrosclerosis 由於腎臟有病加上惡性或急性高血壓而引起， 發病之前都有 benign hypertension。
大部份中小型血管有明顯之 hyperplastic arteriolosclerosis。
malignant hypertension 在血管中主要特徵有二：
(1)eosinophilic granular change 及 inflammatory infiltration 稱為 necrotizing arteriolitis。
(2)interlobular arteries 及 arterioles 的 intima 增厚呈 onion-skin狀，稱為 hyperplastic arteriolitis。
neutrophils 浸潤及 glomerular capillary 有 thrombus稱為 necrotizing glomerulitis。
由於 necrotic capillaries 破裂而產生 hemorrhage。腎小管或是萎縮、或是擴張，其內充滿 hyaline cast。
片子內有一直徑約 5mm 之 cyst 內有 simple cuboidal epithelial lining。
在 cyst 附近靠皮質處有一 2mm 直徑之 microadenoma。由腎小管上皮組成，排列緊密，不呈腺體狀，細胞分化良好。
239. D.M.Nodular glomerulosclerosis, Kidney
D.M. 在 kidney 內，使腎臟血管產生病變，另外還有腎絲球病變，主要有capillaryㄇbasement membrane 變厚，即(a)diffuse glomerulosclerosis,(b)Nodular glomerulosclerosis。Insudative lesion; capsular drops。
micro 下，幾乎每一腎絲球都可見到病變，capillary basement membrane 呈 diffuse thickening，capillary loop 彈性消失看起來很 rigid。有的腎絲球 lobules 甚至呈現 hyaline nodules 又特稱為Kimmelstiel-wilson body。
腎小管有瀰漫性萎縮，或者由於 proteinuria 而其管腔中有 cast。
血管變化明顯。大血管有 atherosclerosis，中型血管有 intimal fibroplasia，再仔細看腎絲球之入球小動脈則可見 hyaline change。
241.Rapid progressive glomerulonephritis (diffuse crescentic GN), kidney
RPGN 使腎功能於很短的時間內衰竭。主要可分三類:anti-GBM, immunocomplex, & pauci-immune。
micro 特徵為腎絲球與鮑氏囊黏連及 parietal endothelial cells 增生造成腎絲球 tuft 對側之鮑氏囊有半月型 lesion 出現。
腎小管中度萎縮，間質纖維化及炎症細胞浸潤都可見。血管有點變化。
243. Renal cell carcinoma, Kidney
片子下一半為 tumor，一半為正常組織。renal cell Ca 為腎小管上皮所衍生。
tumor 主要由 clear cell 及 granular cell 所構成，偶而也會有 spindle cell。本片幾乎為 clear cell。tumor cell 排列成索狀。富血管及腔竇。clear cell 有圓而透亮之細胞值含 glycogen 及 lipids。
245. Neurofibroma, skin
此為一皮下之 neurofibroma，注意週圍沒有 capsule，呈浸潤性生長。
細胞核呈細長或紡錘絲狀，有些類似 schwann cell 之句點狀或波浪狀。
整片之腫瘤細胞埋在 collagen fiber 中。
246. Acute glomerulonephritis (Diffuse endocapillary proliferative glomerulonephritis, postinfectious type), kidney
本片為穿刺切片標本之腎皮質組織。
多數腎絲球都腫大，使 Bowman’s space 消失，而且有 hypercellularity，是由於腎絲球內 endothelial cell 及 mesangial cell 增生，與 leukocyte infiltration 造成。
interstitial edema 及 inflammation。
腎小管有時有 red cell casts 而上皮也有 degeneration 現象。
247. Wilms' tumor (Nephroblastoma), kidney
主要由 mesonephric mesoderm 分化而來，所以 tumor 可由許多不同cell type 所構成，如上皮細胞、muscle、bone、cartilage 等。
此 tumor 有兩種成份：一為似腎絲球之 glomeruloid structure 的上皮性癌細胞，屬於 anaplasia，另一成份為不成熟梭形細胞，於即為blastema cells。
glomeruloid structure 之上皮性癌細胞有橢圓型、嗜鹼性的細胞核。可見 mitosis。
251. Adenocarcinoma of prostate
癌細胞組成之腺體較正常小，其大小、形狀不規則，且由單一層細胞所組成。
浸潤性生長極顯著。
細胞雖小，核比例反而變大且形狀不規則。
癌細胞的細胞質呈顆粒狀，細胞核可為圓形空泡狀。
256. Transitional cell carcinoma, urinary bladder
此種瘤之生長，也是往 lumen 之 papillary growth。
細胞分化不良、多層、loss of polarity，有 mitosis。
此 tumor 並未侵犯到 muscle，故仍為 stage A。
263. Fibroadenoma, breast
主要由 epithelial and connective tissue 二種成份構成，但各別所佔的比例不等。
Fibroadenoma 可分為二種，但可同時存在於一個 lesion 中，這種分法無臨床價值
Intracanalicular--Connective tissue 的增生極快，以致壓迫 ducts 成 slit-like spaces。
Pericanalicular--腺體規則、圓型，外觀仍保持。
Ducts of fibroadenoma--由 cuboidal or cylindral cells with round uniform nuclei 構成，可有 apocrine metaplasia、squamous metaplasia 較少見。
Stroma of fibroadenoma
由 loose connective tissue 構成。
陳年老病，可見 hyaline、calcification，or osseous metaplasia。
Cellularity 變化大。
264. Meningioma, meningotheliomatous type
sheet of tumor cell，長形或橢圓形的核細胞間之界限不清。
有些 tumor cells 會圍繞血管生長，有些圍繞 tumor cells 本身生長，上述兩種情況均稱 whorl formation。
血管有 sclerotic change，此現象及上述 2. whorl formation 可演變成 psammoma body。
psammoma body 為一鈣化物，內主含 hydroxyapatite。
266. Dermoid cyst, ovary
在 ovary 中含有上皮的組織
Skin appendages--Sweat gland, Sebaceous gland, hair follicle and hair
Squamous epithelium
此亦為 teratoma 之一種，因沒有找到 immature element，故可以稱為 mature tearooma。
269. Epidermoid carcinoma, cervix
此 tumor 早已穿過 basement membrane 並往下 invasion，故與 carcinoma in situ 有別。
The tumor is composed of clusters &nests of anaplastic epidermoid cells scattered irregularly in cervix.
有明顯的壞死。
此為 non-keratinized type。
270. Adenocarcinoma, cervix
約佔 cervix carcinoma 的 5%，但機率正逐漸增加中，Gross 與 epidermoid carcinoma 不易區分。
多數是 well-differentiated adenocarcinoma，可有 mucin secretion 也有分化程度差的 tumor。
此 tumor 由分化好的大小不一的 gland-like structure 所組成，可見secretion。
272. Adenomyosis and Leiomyoma, Uterus
Adenomyosis:在 myometrium 中，可以看到有很多散聚的 benign endometrium 之腺體及spindle cell stroma。
Leiomyoma
低倍鏡下，可以看見很多平滑肌細胞，互相交織成旋渦狀的瘤塊，其間交雜著有 connective tissue。
高倍鏡下 tumor cell 大小形狀相近，每個細胞有卵圓形的核及細長雙極的 cytoplasmic process。
anaplasia 及 mitosis 很少見。
有 hyalinization。
278. Chronic myelocytic leukemia (CML), Bone marrow
骨髓細胞增生，以 myeloid series 為主，myeloid cells shifted to left (increase of precursor cells) 。
increase of megakaryocytes, with many atypical forms。
Eosinophilia & Basophilia (切片中無法看出 Basophilia)。
280. Multiple myeloma, bone marrow
multiple myeloma 為 bone marrow 中 plasma cell 有 neoplastic change (monoclonal proliferation)。由於 plasma cell 病變，所以常會產生過量不正常的 Ig，也會造成全身系統性的疾病。
在 bone marrow 部份，則正常的 marrow element 多被已成熟或不成熟的 plasma cells 取代。有的地方還可以見到 giant cell。
282. Malignant melanoma, skin
Melanoma 主要可分為四種，即 lentigo maligna, acral lentiginous,nodular and superficial spreading，而以後者為最常見(佔70%)。
表皮有 ulceration，故有炎症反應。
表皮-真皮交界處被一團團的 atypical melanocytes 所佔有，且向上(表皮)及向下(真皮)浸潤。
Atypical melanocytes 可 epithelioid 或 spindle 形狀，細胞漿中可找到黑色素；此例為 superficial spreading type，細胞呈 epithelioid 樣。
可見明顯的核仁及有絲分裂。
285.Hodgkin's disease, mixed cellular type, lymph node
本淋巴腺局部仍有正常組織存在，如 germinal center、sinusoid 及 capsule。在 lymph node 中到處散布著有 eosinophils、plasma cells、histiocytes 及 lymphocytes (mixed-cellular type 之H.D.)。但 lymphocyte 則比 lymphocyte-predominant type 少。也可以看到散在各種 cell 之間的 Reed-Sternberg cells。
RS cell 為大而核多之細胞，常為雙核，核仁很明顯呈紅色，而核仁外圍著一個較光亮的環，就像是鷹眼(owl-eyed nuclei)。有時也有各種變形如 multinucleated giant cell，單核者稱為 Hodgkin's cells。
289. Hodgkin's disease, nodular sclerosis type, lymph node
fibrosis 嚴重，collagen fibers 分隔 lymphoid tissue，使得 lymph node 呈 nodule 狀。
lymphoid tissue 中，cells 多為 lymphocyte-predominance 或 mixed-cellularity 形態，其中典型的 RS cells 較少，而為 lacunar cells 所取代。lacunar cells 為大的單核細胞，核有時候呈多葉形，核內有細小之核仁，細胞漿多且呈透明或淺紅色。
本淋巴腺物大部為 tumor 取代，只有局部 cortex 仍可見正常之germinal centers。
293. Thymoma, thymus
本例 tumor cells 主要由 thymus 內之 epithelial cells 及 lymphocytes 組成，旁邊包圍著 fibrous capsule，epithelial cells 主要來自 Hassal's corpuscles 邊的細胞。
高倍鏡下 epithelial cell 與一般 cortex 相似，細胞界限不明，有大而空泡狀的(vesicular)細胞核，有的可見核仁。
Lymphocytes 看起來較 active，而且較一般正常在循環系統中的 T cell大。由於 thymoma 是一種牽涉到 T cell 的 tumor，所以，此病常易伴生 systemic autoimmune disease。
295. Follicular adenoma, thyroid
follicular adenoma 可以分成很多種，如 embryonal，fetal，adult，colloid，Hurthle cell 等。視 tumor 中 follicle 大小、數目及intrafollicular stroma 不同而區分。可參考 Robbins。
micro 下，adenoma 的主要特徵有：
Complete fibrous capsule 包圍住 tumor.
clear distinction between the architecture inside and outsidethe capsule.
Compression of the thyroid parenchyma about the adenoma.
The lack of multi-nodularity in the remaining gland.
本片 tumor follicles 緊密而且 follicle proliferation，但大小與正常follicle 差不多，故屬於 adult type。
297. Hashimoto's thyroiditis, thyroid
Hashimoto's thyroiditis 為一種 autoimmune disease。thyroid gland 有時變大，待 epithelium of follicle 都被破壞之後，即產生 hypothyroidism。
大部份的 thyroid follicle structure 都已經破壞而被 lymphoid cell浸潤，包括有 plasma cells，immunoblasts，變形的 lymphocytes 及 macrophage。常形成 lymphoid germinal centers 的構造。
還可見到極少數小的而萎縮之 follicle，有的還有 dense colloid。atrophic follicle 的上皮細胞萎縮，其細胞質含有 brightly eosinophilic granules 稱為 Hurthle cells 或 oncocytes。
有瀰漫性的 fibrosis。
