摘要:

摘要 臨床上許多自聚合式的牙科樹脂其液劑常在高濃度狀態下直接與口腔中的組織接觸,若是牙齦黏膜上有傷口,牙周韌帶纖維母細胞(periodontal ligament fibroblastsPDL)與牙齦纖維母細胞(gingival fibroblastsGF)就有機會與這些液劑相接觸。因此,有關樹脂液劑對此兩種纖維母細胞的生物毒性有必要做進一步的探討。實驗中選擇熱聚合式(Namilon)與自聚合式(Alike)的臨時假牙樹脂液劑各一,二種用於換底墊的樹脂液劑(KoolinerTokuso)二種液劑中的單體甲基丙烯酸甲酯(methylmethacrylate MMA)、甲基丙烯酸異丁酯(isobutyl methacrylateIBMA)及一種交聯劑1,6-hexanediol dimethacrylate (1,6-HDMA)為研究對象。首先以高效能液態層析法得知NamilonAlike的液劑分別含有83.3 %91.3 %MMA單體,Kooliner liquid中含94.5 %IBMA單體,Tokuso rebase liquid中含65.8 %1,6-HDMA交聯劑。接著以MTT (3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide)的方法,探討七種液劑對PDLGF的毒性,並求得各材料的50 %致死劑量。結果顯示純單體IBMA比樹脂液劑Kooliner liquid的毒性強,1,6-HDMATokuso rebase liquid毒性強,但MMA 則比樹脂液劑的毒性要弱。而PDLGF兩種纖維母細胞對每材料在不同濃度下的毒性感受度都很相近。進一步以annexin Vpropidium iodide染色後用流式細胞儀(flow cytometer)分析細胞死亡的方式,發現PDLGF細胞大多數皆朝細胞壞死(necrosis),僅有少數死於細胞凋亡(apoptosis)。以酵素連結免疫吸附定量法(enzyme linked-immunosorbent assayELISA)分析材料影響細胞產生白素-6(interleukin-6IL-6)的情形,結果發現GFIL-6產量比PDL來得高,而且所有材料都有可能刺激GF產生IL-6,但並非每株來自不同病人的GF都會呈現促進的情形,特別的是Tokuso rebase liquid比其他材料更易促進GF產生IL-6。本實驗證實,目前使用之自聚合式牙科樹脂液劑對牙齦組織可能仍有相當的毒性。此結果將有助於臨床使用此類材料的判斷及未來實驗的進行。

   
 

Abstract Autopolymerized resin liquids often contacted with oral mucosa at high concentrations when they were applied in clinics. When there was wound on gingival tissue, periodontal ligament fibroblasts (PDL) and gingival fibroblasts (GF) might be in contact with these resin liquids.Therefore, it is important to understand the cytotoxicity of these resin liquids to PDL and GF. In this study, a heat-curing (Namilon) and a self-curing (Alike) provisional resin liquid, two rebasing resin liquids (Kooliner, Tokuso), two monomers (methylmethacrylate [MMA], isobutyl methacrylate [IBMA]), and one cross linking agent (1,6-hexanediol dimethacrylate [1,6-HDMA]) were invastigated. The concentrations of monomer or the cross-linking agent in resin liquids were detected by high performance liquid chromatography (HPLC). There were 83.3 % and 91.3 % of MMA in Namilon and Alike liquids, respectively. Kooliner liquid contained 94.5 % of IBMA, and Tokuso rebase liquid contained 65.8 % of 1,6-HDMA. The effects of test materials on viability of PDL and GF were determined by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide (MTT) assay. The results suggested that IBMA and 1,6-HDMA were more toxic than Kooliner liquid and Tokuso rebase liquid, respectively. However, MMA was less toxic than Namilon and Alike liquids. The sensitivity of PDL and GF had similar response to each testing materials at different concentrations. Analysis of double staning with annexin V and propidium iodide  demonstrated that the majority of cells died by necrosis, and only a small portion of cells were by apoptosis. The IL-6 production as determined by the enzyme linked-immunosorbent assay (ELISA) of GF cells from some doners, were enhanced by all of the test materials. All of the test materials might enhance the production of  interleukin-6 (IL-6) by GF cells derived from some donors. However, differences may exist when GF cells from different subjects were analyzed. The information obtained in this study is of value for clinical judgement in using dental resins and future invastigations.
