摘要:

摘要 嚼食檳榔的人發生口腔黏膜下纖維化症(oral submucous fibrosis)、口腔白斑症(oral leukoplakia)與口腔癌(oral cancer)的機率比未嚼食檳榔者來的高。也有研究指出,嚼食檳榔者比未嚼食檳榔者可能有較高的牙周病患率。當牙周組織受細菌感染時,早期主要防禦者為嗜中性白血球(neutrophils),若口腔中的中性白血球殺菌功能降低,可能使致病細菌容易在口中繁殖而侵襲牙周組織。本研究之目的在探討檳榔嚼塊成分對嗜中性白血球吞噬作用的影響。所用檳榔嚼塊成分有:成熟去殼之檳榔子萃取物(ripe areca nut extractripe ANE)、未成熟含殼之檳榔子萃取物(tender ANE)、檳榔素(arecoline)、兒茶酚[(+)-catechin]、丁香(eugenol)與黃樟素(safrole)。首先將不同成分分別處理中性白血球30分鐘後,以錐藍質排除測試(trypan blue exclusion test)分析,結果得知所使用的濃度並不影響中性白血球的存活,而後將伴放線桿菌(Actinobacillus actinomycetemcomitans)及突變形鏈球菌(Streptococcus mutans)標示上螢光,與經檳榔嚼塊成分分別處理後之嗜中性白血球進行吞噬作用,藉由流式細胞儀(flow cytometer)分析中性白血球吞噬含螢光細菌的情形,結果得知ripe ANE (50 mg/ml)tender ANE (720 mg/ml)(+)-catechin (4000 mg/ml)safrole (10 mM)使嗜中性白血球吞噬A. actinomycetemcomitans的能力降低至約50,而arecoline (20000 mg/ml)eugenol (5 mM)降低吞噬作用至約5859ripe ANE (40 mg/ml)arecoline (12000 mg/ml)(+)-catechin (5000 mg/ml)safrole (8.2mM)使嗜中性白血球吞噬S. mutans的能力降低至約50,而tender ANE (800 mg/ml)eugenol (5 mM)分別降低中性白血球的吞噬作用僅至5982。進一步以共焦顯微鏡(confocal microscope)觀察吞噬作用,結果得知,嗜中性白血球吞噬能力確實有降低的現象。由此可知,檳榔嚼塊之成分可以降低中性白血球吞噬功能,而可能藉此影響口腔健康。

   
 

Abstract    Area quid (AQ) chewers have a higher prevalence of oral submucosal fibrosis (OSF), oral leukoplakia and oral cancer than non-chewers. A higher prevalence of periodontal disease among AQ chewers than non-AQ chewers has also been shown. When the periodontal tissue is infected by bacteria, neutrophils are the major defender during the early stage of infection. When the bactericidal ability of  neutrophils is reduced, the numbers of oral bacterial pathogens may be increased and the pathogens may invade the periodontal tissue. The goal of this study was to investigate the effects of the ingredients of AQ on phagocytosis of neutrophils. Ripe areca nut extract (ANE), tender ANE, arecoline, (+)-catechin, eugenol and safrole were used. Treatment with various components of AQ for 30 minutes had no cytotoxic effect on neutrophils as determined by a trypan blue exclusion assay. Actinobacillus actinomycetemcomitans and Streptococcus mutans were labeled with fluorescence. The effects of various components of AQ on phagocytosis of fluorescent bacteria by neutrophils were determined by flow cytometer. Approximately 50% of the phagocytosis was affected when 50 mg/ml of ripe ANE, 720 mg/ml of tender ANE, 4000 mg/ml of (+)-catechin or 10 mM of safrole was used. However, 2000 mg/ml of arecoline and 5 mM of eugenol reduced the phagocytic activity of neutrophils to approximately 58% and 59 %, respectively. For S. mutans, approximately 50% of phagocytosis was affected when 40 mg/ml of ripe ANE, 12000 mg/ml of arecoline, 5000 mg/ml of (+)-catechin or 8.2 mM of safrole was used. However, 800 mg/ml of tender ANE or 5 mM of eugenol was required to reduce the phagocytosis of neutrophils to 59% or 82%, respectively. Moreover, the reduction of phagocytosis of neutrophils was observed by the confocal microscope. In conclusion, various components of AQ affected phagocytosis of neutrophils, which may affect oral health.
